Adult Home Care Program 937-225-4500
• Provides skilled nursing, physical and speech therapy, medical social worker & nutrition services
• Requires a physician order and must be a Montgomery County resident
• Focus on those who are uninsured or under-insured
• Pediatric Physical Therapy Program
• Provides assessments & treatment of infants & toddlers ages 0-36 months at risk for developmental concerns
• Requires a physician order, accepts private insurance, Medicaid Managed Care, and Early Intervention System of Payment
• Coordinates care with other providers contracted thru Help Me Grow/Brighter Futures
Baby & Me Tobacco Free 937-496-3376
• Free smoking cessation program for pregnant women
• Open to current smokers or those who quit smoking within three months of becoming pregnant
• Diaper vouchers given to those that successfully participate in the program

• Referrals for substance abuse treatment and other health & social services
• Condom distribution
• Assistance with Medicaid sign-up
Children with Special Healthcare Needs (CMH) 937-225-6459
• Public Health Nurse services for families with children who have special health care needs from birth to age 21
• Help finding and applying for services needed for your child, community resources, and health education
Chronic Disease Home Visiting Program 937-225-5700
• Provides one-on-one education and coaching for those living with a chronic disease, in the comfort of the participant’s home
• Focuses on informing individuals how to improve current chronic diseases, prevent the onset of future diseases, and manage their conditions long term.

• Identify chronic health problems and provide referrals

• Provides the tools needed to successfully control diabetes and prevent complications
• Offers a prevention program for those at risk for type 2 diabetes
